If you’re interested in becoming a new patient, please call our office at (773) 296-6700 during office hours. If we miss you, please leave a detailed message that includes your name, phone number, and the name of the practitioner you’d like to see. We make every effort to reach new patients as soon as possible.

In order to better serve you, here’s a quick checklist of items to prepare before your first visit:

• Download the New Patient Forms, complete and sign them, and bring them to your first appointment. If you aren’t able to print them out, please arrive for your appointment 30 minutes early so you can complete them.
• Confirm coverage for your appointment with your insurance company.
• If you are coming in for a non-covered service, please bring payment in the form of Visa, Mastercard, Discover, American Express, check, or cash. Payment is due at the time service is rendered.
